This declaration is to apply to all donations I have made since 6^th April 2000 and to all donations I make hereafter. I want all of these donations to be treated as Gift Aid Donations.

I understand that I must pay an amount of income tax or capital gains tax equal to the tax HOPE worldwide reclaims on my donations (currently 28p for every £1 I give).

I understand that signing this form does NOT impose any restriction on me cancelling future planned payments to HOPE worldwide at any time. I also understand that signing this form does not cost me any more money over and above what I regularly contribute to HOPE worldwide.
